Note! Children under the age of 12 can only have their blood drawn at the hospital. An appointment is required for this.

What do you need to bring when having your blood drawn?

• A valid ID (required)
• The lab test request form

Received a barcode and Zorgdomein number? Then you do not need a request form.
Your barcode will be scanned, and we can see your request form.
